Overview

Each 4-H program, activity, or event may require different paperwork to make sure all needed information is collected, safety and emergency protocols are clarified, and guidelines for behavior are clear.  Always contact your county extension office for more information (link) and to get needed paperwork.  The two forms below are usually consistent from program to program.

Curriculum

• 4-H Participant Information Enrollment Form and Code of Conduct (Required for all youth participants)
• DOC File
• PDF File
• Spanish

• Photo Release Form (This is included as part of the 4-H Participant Information Enrollment Form)
